Memorials form

Friends of the Wild Flower Garden, Inc.

Thank you for considering a Memorial to the Friends of the Wild Flower Garden in support of the Eloise Butler Wildflower Garden and Bird Sanctuary.
You have two options for making a Memorial. 1. Fill out our Memorial form as shown below and mail with your check, payable to "The Friends of the Wild Flower Garden," to our address on the form. To do so, print out a copy of just the form by using the "Memorial form" link at the right.	Printable Memorial Form
2. Complete the Memorial process on-line and pay by credit card or bank transfer. If you wish to make a Memorial and pay via online credit card, bank transfer or your PayPal account, click the donate button at right. You will be transferred to our secure payment processor, which is PayPal, where you may chose your method of payment. You do not need to be a member of PayPal to do this, nor will PayPal retain any of your information. When you have made payment you will be returned to The Friends web site. After entering your payment information and before confirming your transaction, you will see an information box into which you type information about the memorial. Please also give us the name and address of the person that should be notified that you have made a memorial.	On-line Payment
	Memorials will be acknowledged by thank you note to the donor and by notification about the Memorial to a person you designate. All Memorials and other donations are tax deductible. Memorials are also acknowledged (names only) in The Fringed Gentian unless you instruct us otherwise.
	Memorial Honor Board
	On the inside wall of the Martha Crone Visitor Shelter hangs the Eliason Memorial Board, presented by Marian Eliason in honor of her mother, Lydia. An engraved name plate will be attached for Memorials for a deceased person when the amount received reaches $100. A name plate will be attached for Memorials in honor of a living person if the amount is above $500.
